So as to help as many people as possible, the attached Certificate in Lieu of W-4 is provided for those who are employees. This certificate meets title 26 guidelines and is the ONLY form the IRS does not provide, because it's purpose is to stop employers from withholding income tax and social security. This is the form that employees submit, as the title indicates, in lieu of a W-4. All of the title 26 statutes that apply are on the form.

In the form i am confused on how to determine the number of withholding exemptions you are claiming.
Yeah, I can see where this might look confusing. The idea was to make the certificate look as much like the government version as possible. The correct answer is one. You are exempting yourself from withholding. You will note that you are filling this out for a specific year. So if you are filling this out for this year, you would fill in 2015 for the year. It is best to fill this out in December for the following year so that your employer has it on file before your first paycheck comes in.
This is the only form you can't get from the IRS for obvious reasons. The so called law states that your employer is supposed to make this form, probably because most would not do it. So if you fill it out and take it to your employer you may get some resistance, but most will just look at it and think it official, which was the goal. It actually is official meeting the specific guidelines of the IRS.
I would recommend you read the articles under the IRS menu to familiarize yourself with the info also. It will bolster your knowledge.

We can use this no matter where we work, even if it's for a government contractor? We will not be audited by the IRS for submitting this? I'm very new at all this, so I'm trying to learn. Thank you!

Makes no difference. Title 26 is not law anyway. So send this to your employer, then send the IRS the affidavit from forum. They can't audit you, because you cannot be required to show them anything, because that is testimony against yourself. If the IRS tries to force the issue, you can file an action against the perpetrators. I will be filing a case against them myself once I have time to write it up.

The last time I tried to offer a W4 (not the "in lue of..."), the employer used an electronic web based payroll and I tried to use 000-00-0000, that didn't work. I see that a S.S. # is given, so I guess I would get past that hurdle, and the additional info. on ILOW4 informs the employer not to make deductions per the code.
Do you know how many of the members have used this, and are there any additional tips?
Also, with Ca. you are suppose to file a separate return. When the employer entered no deductions for the iloW4, they entered the same answers for the state info. Does that sound legit to you experts? LOL

I work for a sporting goods corporation and am wondering if I notice the irs with a letter stating that according to title 26 USC 7806 I'm not a tax payer while filing the in lieu of w-4, do I file my returns next spring for the taxes I've paid so far in 2018, or do I just not worry about it once my filing status has been adjusted with the IRS since I won't be paying taxes from now on?
